Enrollment Agreement for Parts I & II

CFL refers to Coach For Life Institute throughout.

**PART I & II COURSE REQUIREMENTS

All Pre-requisites must be met before a registrant's space is reserved.

Pre-requisites:

1. Verifying that you can participate in a webinar conducted in English and hosted by ZOOM before paying your Non-Refundable tuition for this course.
2. Agreeing to have or create a Gmail account to facilitate participation in a Google Classroom.
3. Giving permission to Coach For Life Institute to
   • Share your name and email address(es) with the ICF for credentialing records as well as to receive a completion survey from ICF.
   • Record class sessions where registrant is a class participant, coach and/or a coaching client. (Recordings with student participation to ONLY be shared for learning & accreditation purposes with other class participants, CFL facilitators, CFL class mentors or submitted to ICF for accreditation review.)

** PARTS I & II - ATTENDANCE & COMPLETION REQUIREMENTS to Receive Course Credit

Part I 4-Day Webinar

• Must be attended LIVE and participate in all sessions (LIVE on camera).
• All class work must be completed within 2 weeks of last class session to receive any credit for Part I.
• Missed sessions - Pay $50 administration fee & attend another occurrence of "like" LIVE session to get credit.

Part II Webinar Sessions

• Course credit can be earned by listening to the recording and submitting the required class work.
• All class work must be completed within 2 weeks of last class session to receive any credit for Part II.

**PARTS I & II FACILITATORS
Coach For Life Institute cannot guarantee specific facilitators for the group.
Changes in scheduled facilitator(s) does not qualify as a reason for a transfer nor a refund.

**PARTS I & II PAYMENT, TRANSFER & CANCELLATION TERMS
All Payments are Non-Refundable & Non-Transferable.

• Payments made to CFL are by credit card.
• Any Balance Due Payment will be processed on the same credit card ~35 days before the start date of this Part I 4-Day Program. Exact date for payment will appear in registration email.
• If registering within 35 days before the start date of this Part I 4-Day Program, any Balance Due will be billed to the same credit card within 24 hours.
• If full payment is not received by 32 days before the start date of this Part I 4-Day Program
  • Your $200 reservation deposit is forfeited and
  • Your reserved space is canceled and made available to another registrant.

I further declare that I have read, understand and accept Coach For Life Institute's Company Policies and Terms & Conditions on the COMPANY INFO menu here. If any discrepancies or inconsistencies exist between the Company Policies and Terms & Conditions posted on the Coach for Life Institute website, this course's Enrollment Agreement will take precedence for this course.
