ENROLLMENT AGREEMENT FOR LEVEL 1

CFL refers to Coach For Life Institute and/or Coach For Life Education throughout.

**LEVEL 1 COURSE REQUIREMENTS

All Pre-requisites must be met before a registrant's space is reserved.

Pre-requisites:

1. Verifying that you can participate in a webinar conducted in English and hosted by ZOOM before paying your Non-Refundable deposit for this course.
2. Agreeing to have or create a Gmail account to facilitate participation in a Google Classroom.
3. Giving permission to Coach For Life Institute & Coach For Life Education to
   • Share your name and email address(es) with the ICF for credentialing records as well as to receive a completion survey from ICF.
   • Record class sessions where registrant is a class participant, coach and/or a coaching client. (Recordings with student participation to ONLY be shared for learning & accreditation purposes with other class participants, CFL facilitators, CFL class mentors or submitted to ICF for accreditation review.)

** LEVEL 1 - ATTENDANCE & COMPLETION REQUIREMENTS to Receive Course Credit

Part I 4-Day Webinar

• Must be attended LIVE and participate in all sessions (LIVE on camera).
• All class work must be completed within 2 weeks of last class session of the 4-day webinar to receive any credit for Part I.
• Missed sessions - There are no other sessions of this nature being offered by Coach For Life Institute so no make-up option is available.
  • Participant receives CCEs for actual live attendance hours completed.

Part II & Part III are combined into Nine 2-Hour Weekly Webinar Sessions

• Six of the Nine 2-hour sessions are coach's training for Building a Coaching Practice
• Three of the Nine 2-hour sessions are Group Mentoring sessions

BE SURE TO KNOW WHETHER A 2-HOUR WEEKLY SESSION IS A MENTORING OR A TRAINING SESSION.

• MENTORING SESSIONS (Part III)  Must be attended LIVE and participate in all sessions (LIVE on camera).
  • Mentoring session dates are July 10, July 24 & August 7
• TRAINING SESSIONS (Part II) Can be completed by listening to the recording & completing all assignments for that session
  • Training session dates are June 26, July 3, July 17, July 31, August 14, & August 21

Part II 2-Hour Training Webinar Sessions

• For Part II ONLY - Course credit can be earned by attending live or listening to the recording & completing all assignments for that session
• All class work assigned within the training sessions must be completed within 2 weeks of last class session to receive any credit for Part II.

** PART III - ACLC ATTENDANCE & COMPLETION REQUIREMENTS to Receive Course Credit

Part III 2-Hour Mentoring Webinar Sessions

• Must be attended LIVE and participate in all sessions (LIVE on camera).
  • Mentoring session dates are July 10, July 24 & August 7
  • Any missed group mentoring session can ONLY be made up by scheduling and paying for two 1-hour individual mentoring sessions.
    • Current rate for 1-hour mentoring session is $150 so making up a 2-hour group mentoring session is $300
• At least 3 group 2-hour sessions, 3 individual 1-hour sessions plus Coach-Like Conversations must be attended & participated in LIVE to receive credit for the course.

Part III additionally requires 3 one-hour Individual mentoring sessions -

• • 3 Individual one-to-one mentoring sessions are included in the tuition for this course to meet ICF requirements
  • Must be scheduled individually at 2-week intervals to meet ICF requirements
  • You are responsible for scheduling with an available designated CFL mentor
    • The final one-to-one mentoring session must be scheduled between August 24 and August 31, 2024
  • The coach can use the same client for all three individual sessions or the coach can use a different client for each individual session
  • For each different client you record you must submit:

1. Client Authorization Letter (template provided by CFL) signed by the client that they, the client, has agreed to allow the coach to record their coaching session.
2. Provide recording & Self-Discovery sheets of both coach & client at least 4 hours before scheduled individual mentoring sessions.
3. Additionally a transcript of the recording must be submitted at least 24 hours before scheduled individual mentoring sessions.
4. Recorded coaching sessions must be 25-35 minutes from hello to goodbye – without editing.
5. The last of these 3 sessions will be used for the Final Performance Evaluation.
6. A minimum of 24-hour notice is required to reschedule an individual mentoring session.
   • Failure of sufficient notice requires signing up & paying for additional individual mentoring sessions for the then current fee posted on the website - (currently $150 per one-hour session) until you complete the 3 required individual sessions.

You can sign up for additional one-on-one Private Mentoring sessions through CFL for the then current fee posted on the website - currently $150 per one-hour session.

** PART III - ACLC FINAL PERFORMANCE EVALUATION PROCESS

• All 10 hours of mentoring must be completed before the Final Performance Evaluation processing can be started with CFL.
• Coach For Life Institute has been ICF Level 1 accredited and can perform the Final Performance Evaluation as well as issue a Level 1 certificate..
  1. The recording, transcript & Self-Discovery sheets of both coach & client from your last individual mentoring session will be used for the ACC-level Final Performance Evaluation.
  2. Additionally, you must provide CFL’s mentor’s written ACLC Assessment of that coaching session.
  3. Must successfully PASS the ACC-level Final Performance Evaluation completed with CFL at or above the ICF ACC-level Criteria.
  4. If you receive a FAIL on the ACC-level Final Performance Evaluation completed with CFL you can apply again for the ACC-level Final Performance Evaluation completed with CFL by
     • Signing up & paying for additional individual mentoring sessions as you need until you feel ready to apply again. Then when ready -
       • • Pay the then current fee posted on the website - currently $100 application fee for the ACC-level Final Performance Evaluation completed with CFL
         • Signing up for and completing a new one-on-one Private Mentoring sessions with a CFL Mentor for the then current fee posted on the website - currently $150 for a one-hour session.
         • Provide recording, transcript & Self-Discovery sheets of both coach & client from one of your individual mentoring sessions.
         • Must successfully PASS the ACC-level Final Performance Evaluation completed with CFL at or above the ICF ACC-level Criteria.

**LEVEL 1 FACILITATORS
Coach For Life Institute cannot guarantee specific facilitators for the group.
Changes in scheduled facilitator(s) does not qualify as a reason for a transfer nor a refund.

**LEVEL 1 PAYMENT, TRANSFER & CANCELLATION TERMS
All Payments are Non-Refundable & Non-Transferable.

• Payments made to CFL are by credit card.
• Any Balance Due Payment will be processed on the same credit card ~35 days before the start date of this 4-Day Program. Exact date for payment will appear in registration email.
• If registering within 35 days before the start date of this 4-Day Program, any Balance Due will be billed to the same credit card within 24 hours.
• If full payment is not received by 32 days before the start date of this 4-Day Program
  • Your $200 reservation deposit is forfeited and
  • Your reserved space is canceled and made available to another registrant.

I further declare that I have read, understand and accept Coach For Life Institute's Company Policies and Terms & Conditions on the COMPANY INFO menu here. If any discrepancies or inconsistencies exist between the Company Policies and Terms & Conditions posted on the Coach for Life Institute, LLC website, this course's Enrollment Agreement will take precedence for this course.
